Part B
There is a 10% penalty for each year you could have enrolled but didn’t, and you may also pay a higher premium based on your income. However, you usually won’t have to pay a penalty if you qualify for a Special Enrollment Period. If you’re under 65 and disabled, the penalty ends when you turn 65 because you’ll have another Initial Enrollment Period. In most cases, you’ll have to pay the penalty every month for as long as you have Part B.
Part D
The penalty is calculated by multiplying 1% of the national base beneficiary premium by the number of months you were eligible but didn’t enroll. In 2024, the national base beneficiary premium is $34.70, so the penalty is 1% of that, or $0.3470, multiplied by the number of months. The final amount is rounded to the nearest $0.10 and added to your monthly premium. The penalty amount may increase each year because the national base beneficiary premium may increase. After you enroll, your plan will tell you if you owe a penalty and what your premium will be.
